The Arete Health Approach to
Advanced Specialty Laboratory Diagnostics

Thank you for your interest in scheduling an Annual Wellness Physical at Arete Health.
Please read and understand all the information below before deciding to submit your application.

For all specialty and supplemental services, we require all of our patients to be under the direct care of a local or Primary Care Physician (PCP) who manages day-to-day routine and urgent care.

Dr. McCloskey’s Functional Medicine private practice is limited to a very small number of patients. It takes tremendous time and energy for any healthcare worker to manage a complicated case. The patients who typically do well working with Dr. McCloskey are highly motivated and dedicated to evaluating their case in great depth. Successful management of such cases requires appropriate testing, diagnosis, financial commitment, and realistic clinical expectations.

INSURANCE
We provide a variety of testing services, and some of them may or may not be covered by your insurance plan. Arete Health and Palm Beach Functional Medicine do not directly partner with any insurance providers. For most testing, we can provide patients with insurance codes to submit for insurance purposes for potential reimbursement from their insurance companies, although this is not guaranteed.

Dr. McCloskey has a private Functional Medicine practice in West Palm Beach, Florida. To become a patient under his care, you must undertake the two-step process outlined below.

Step 1: Initial Consultation
Dr. McCloskey has a limited clinical practice and is growing in demand domestically and internationally. In response to the increased number of patients requesting consultations, he implements a case review process to determine whether he is the right provider to support your health goals. This helps you avoid unnecessary travel time and expenses.

To be scheduled for a case review you must complete our Ensights Application found here, unless you have been provided a separate link by a partnering provider.

Once you submit your application, your patient advocate will review your materials and let you know if any additional information is required. Once your application is approved, we will reach out to schedule you for your case review appointment. Once your case review is scheduled, you will be sent an Ensights welcome package including several health questionnaire forms, and a detailed medical history. You will also need to submit any recent and relevant medical records, including imaging and laboratory results. Due to Dr. McCloskey’s limited schedule, only once an application is approved can we schedule the case review consultation.

In preparation for the case review, Dr. McCloskey will review this information in detail and prepare comprehensive notes. The case review consultation is 60 minutes and can be scheduled in-person or via video teleconference. Directions can be found here.

The case review consultation is a stand-alone visit - nothing more is required. The consultation is not a guarantee of becoming a patient at our clinic; during the consultation, you and Dr. McCloskey will discuss your case and decide if he is your best choice as a healthcare provider. If so, the next step is scheduling your in-person comprehensive physical examination. Due to the time-intensive process of reviewing a patient’s extensive application materials, and preparing for and conducting a case review, there is a flat fee of $300.

The case review serves multiple purposes:

  • The case review allows Dr. McCloskey to review your health history and medical records prior to determining appropriate patient-fit prior to your examination and additional laboratory diagnostics.

  • The case review gives you an opportunity to meet him in person and ask specific questions about becoming a patient.

  • The case review will provide a clearer sense of the total cost involved in managing your case.

  • The case review will provide important information regarding informed consent, including: a) the name of the test(s) or procedure(s), b) the reason you are having the test(s) or procedure(s), c) what results to expect and what they mean, d) the risks and benefits of the test(s) or procedure(s), e) possible side effects or complications of testing and/or interventions, f) when and where you are to have the test or procedure, g) who will be performing the test(s) and/or procedures and their qualifications, h) possible/probable health consequences of opting out of testing, i) alternative options for testing and procedures, j) when and how you will get the results, k) who to contact after the test(s) or procedure(s) if you have any issues, questions, or concerns, l) and how much you will have to pay for the test or procedure.


Specific treatment recommendations are not discussed during the case review consultation. Only after having done the in-person physical examination and laboratory diagnostics is Dr. McCloskey able to create your customized care plan.

Step 2: Laboratory Diagnostics Specimen Collection
The new patient evaluation is the point at which the formal practitioner-patient relationship begins. After the case review consultation, Dr. McCloskey requires each new patient travel to his clinic in West Palm Beach, Florida for an in-person new patient exam. He will not begin with a new patient on a remote basis. For out-of-town patients, a travel guide, including hospitality options, food, shopping, and activities, will be provided to you by our patient coordinator.

The examination is a rigorous evaluation and report of findings requires 3-4 hours of your time in the clinic. Dr. McCloskey will also spend approximately 6-10 hours before and after the appointment to develop a treatment plan, after he has reviewed your health history, labs, new patient findings, and any additional new findings. The cost of the new patient examination and laboratory diagnostics package begins at $3,000. The exact cost will be determined during your case review based on your specific case and goals.

Prior to your new patient examination appointment, we will provide complimentary sample collection services in-house for your convenience. We ask international patients to schedule a few extra days in the area to take advantage of our specialty testing accessible only in the United States.

Step 3: Follow Up Appointment Including Report of Findings + Professional Health Coaching Session
Follow up appointments with Dr. McCloskey are scheduled for video teleconference or in-office, depending on your preference, location and needs. After the initial examination, a patient is not required to return to the clinic except for future re-examinations.

Follow up health coaching appointments are scheduled for 120 minutes at a cost of $300 each. Most patients typically need eight to twelve follow up appointments in the first year. All appointments are scheduled in Eastern Standard Time (EST).

Lab testing could run between $500-$5,000 during the course of the first year. We do not provide insurance codes or insurance-related receipts for any medical testing that is arranged and billed through our office. Certain lab companies will bill your insurance on your behalf.

Nutritional supplementation costs may run between $1,000 and $3,000 during the course of the first year. The clinic does not hold supplement inventory, however we will send you a link to one of our virtual dispensaries for you to place your order with direct shipping.

Ongoing Care
The overall costs for the first year of care usually run between $5,000-$10,000. Any continuing care, if required, is substantially less expensive since less resources are required to evaluate and manage your continued progress or maintenance. Case management and costs are different for each patient depending on their unique needs. Typically, local patients will opt-in for a yearly wellness physical and/or yearly wellness labs for continuous surveillance of important biometrics, while some patients opt for regularly scheduled, ongoing health coaching consultations.
