Becoming a Patient

  • To get started on the healing journey, we ask you begin by filling out the Prospective Patient Application and tell us about your health concerns.

    Once you fill out the application, a patient care coordinator will be in touch with you within a few days to make sure that the care we provide fits your health needs.

    1. Functional Medicine Consultations: 

    Your health journey will begin with an initial consultation over the phone with a Physician Assistant to learn more about your health history and background. Next, you will meet with Dr. Jason McCloskey by traveling to our clinic in West Palm Beach, Florida. Each new patient experience is designed to cover everything from an extensive health intake, through innovative lab testing and nutrition consultations.

    2. Functional Nutrition Consultations:

    Functional Nutrition is a wonderful entry point to Functional Medicine. Working with our nutritionists is a great way to get started on your health journey. 

    Our nutritionist can run Functional Nutrition lab testing on you so they can personalize your diet. In this blog post you can learn more about how Functional Nutrition could potentially help you heal.

    3. IV Therapy:

    We offer intravenous therapies that will support your general wellbeing, treat nutrient deficiencies, as well as support your healing journey at The UltraWellness Center. While we strongly believe in a food-as-medicine approach to healing, we also understand the value a “flood of nourishment” provides to patients healing from chronic illness and those seeking a return to optimal health. IV infusions and nutrient injections filled with vitamins, minerals, and other nutrients can often be the turning point to spark a reversal of symptoms for those with chronic illness, or those looking to optimize their health and performance.

    4. Ozone/Prolozone Therapy:

    Oxygen-ozone therapy has a long history of safe use to support and improve health. It can support the body to activate its own healing mechanisms, including inhibiting inflammatory pathways, activating anti-inflammatory pathways, and increasing your body’s own antioxidant and detoxification systems to improve overall health.

    Prolozone therapy is a safe and effective way to address chronic musculoskeletal complaints and support the regeneration and repair of joints and tissues.

  • Yes, of course! All of our health coaching services are virtually capable. For years we have been consulting with people from around the world via video teleconference. For patients looking for comprehensive care, an initial in-person physical examination is required, however all follow-up appointments can be scheduled via video-teleconference at your convenience. All appointments will be scheduled in Eastern Standard Time.

  • To work with a Functional Medicine physician you will be required to come to The UltraWellness Center in Lenox, Massachusetts for your initial consultation and maintain a relationship with a primary care physician.

    Follow-Up appointments can be done remotely; however, patients are required to be seen in-office on a yearly basis.

  • Patient Guidelines for Medical Records Submission
    As you begin your Functional Medicine journey with us, we begin a relationship together that will take course over the months and/or years to come. As inherent in the Functional Medicine model, we will want to understand the complexity of who you are, your story, the triggers and precipitating factors that have impacted your health, and where this has brought you to date. This discovery process is part of us getting to know you, your history and your needs, and helps us formulate a treatment plan that is specifically catered to you. There are many facets to this initial discovery process. You will be asked to complete an online questionnaire upon scheduling your appointment. You may be asked to have a Pre-Visit appointment with a Physician Assistant so we can focus more specifically on the information needed for your first appointment with the doctor. To better help you understand what medical records are useful, we have outlined the following guidelines:

    • Medical summary within the last 12 months from your Primary Care Provider

    • Conventional lab results within the last 3-6 months

    • Functional Medicine test results within the last 12 months

    Items we ask that you DO NOT SEND:

    • Notes from routine medical office visits

    • Routine exam notes (this includes immunizations, routine physical exams, etc.)

    • Lab results that are over a year old

    • Diagnostic imaging films or disks

    Please do not submit screenshots of your medical records. We require copies of your original medical documents.

    * While we recognize that everyone is unique in the journey that has brought you to us, so can this apply to the information that may be pertinent. Please remember, more is not always better. If you request multiple past medical records to be reviewed by our physician, an additional fee will be applied.

  • Dr. McCloskey works as a functional medicine practitioner in conjunction with your current health care team and is not considered a replacement for your primary care physician. He requires all new patients to have a primary care physician to cover emergencies and routine care and screening.

    Dr. McCloskey is a Doctor of Chiropractic (DC) with additional post-doctoral education in clinical neuroscience and functional medicine. He received his doctorate from Parker university in Dallas, Texas. He does not prescribe pharmaceutical drugs and does not diagnose or treat medical conditions, instead focusing on promoting health and optimal function. His services are not meant to replace those of a medical doctor and instead work in conjunction with them as part of an allied healthcare team.

  • No. He does not prescribe or make recommendations about prescription medications or controlled substances. Your prescribing medical doctor will manage all medications. Dr. McCloskey uses natural, noninvasive methods such as movement therapy, nutritional therapy, herbs, supplements, stress management technologies and lifestyle changes to address the underlying causes of disease and promote optimal function and health.

  • No, we require that you have your own primary care physician. While our physicians are trained as primary care physicians, they do not provide acute care services. We will work with you closely as consultants and coaches in preventive, nutritional and functional medicine to help you address the roots of chronic health problems. They can confer with your primary care doctor if required.

Supplements

  • The FDA does not approve dietary supplements for any purpose. Products that have claims to treat, diagnose, prevent, or cure diseases are generally subject to regulation as drugs. Dietary supplement companies are responsible for ensuring that their products are safe and accurately labeled, and their products are regulated by the FDA, but much of our role begins after products enter the marketplace. In fact, in many cases, companies can produce and sell dietary supplements without even notifying the FDA. The FDA works to ensure that dietary supplements meet applicable safety standards and that they are well-manufactured and accurately labeled. The FDA is further responsible for enforcing the laws and regulations governing dietary supplements.

  • Physicians and practitioners who do not wish to enroll in the Medicare program may “opt-out” of Medicare. This means that neither the physician, nor the beneficiary submits the bill to Medicare for services rendered. Instead, the beneficiary pays the physician out-of-pocket and neither party is reimbursed by Medicare. A private contract is signed between the physician and the beneficiary that states, that neither one can receive payment from Medicare for the services that were performed. While this might seem like a drawback for patients reliant on Medicare, there can be several key benefits for patients in this arrangement:

    Providers who opt out of Medicare often have more flexibility in the care they offer and the amount of time they can spend with each patient.
